Chelsea manager Jose Mourinho cut short his Premier League celebrations because he went to the hospital to visit Ramires.
The midfielder was set to start the crucial game against Crystal Palace, but he was taken ill before the match.
The Brazil international was removed from the starting line-up just before kick-off and then taken to hospital.
Speaking after the match, Chelsea manager Jose Mourinho said: “Ramires went to the hospital; he was feeling so bad.”
The player is likely to suffer from kidney stones, but Chelsea haven’t confirmed it yet.
Ramires was replaced by Juan Cuadrado in the starting XI, and Chelsea went on to beat Crystal Palace 1-0 thanks to Hazard’s goal, securing their status as Premier League champions.
